Please remember to bring the following to your appointment:

• Your referral letter from your GP or referring doctor
• Medicare or Veterans Affairs card
• Health insurance information
• Copies of any relevant medical information/documentation
• Please arrive 5-10 minutes early for your appointment for registration and to fill in a Patient Registraion Form. Alternatively you can download it, fill it out and bring it with you on the day.

Our Practice is open from 8:30am – 4:30pm Monday to Friday, except on Public Holidays.
